TAKE CHARGE PROVIDER
Take Charge is a program funded by the federal government that covers birth control methods and birth control related office visits. You can apply for Take Charge at Aurora Medical Services. To qualify for Take Charge, you must:
- have no other form of health insurance.
- be a US citizen or permanent legal resident for five years or more
- meet certain income criteria, listed below
- provide a valid social security number
- provide a photo ID
- provide a copy of your birth certificate or passport
- live in Washington State
The following table shows the most money you can make before taxes for your family size and still qualify for Take Charge.
